Eckstein Band Event Forms
and Information
Find many of the forms you need for ordering band shirts, signing up for the band email list, and attending band events and activities (listed on the Event Schedule) below. To print a form, click on the appropriate form link to access the PDF version of the form.
Complete the form and return it to the appropriate teacher or event coordinator (specified on the form). At this time, you cannot complete and submit the form online.

2007 - 2008 Eckstein Band Apparel
This is your final chance to order your 2007 - 2008 Eckstein Band Apparel! Orders are due by January 31, 2008. Please complete the Band Apparel Order Form and return with your payment to the mail box in the Band Room or to Susan Karboski (see address on the order form).

Participants, please complete the appropriate form(s) and return it to the address located on the bottom of the form or hand deliver it to the Mail Box in the Eckstein Band Room.
Both the Solo Entry Forms and Ensemble Entry Forms must be post marked by or hand delivered to the mailbox in the Eckstein Band Room by Friday, February 15th, 2008.
No late entries will be accepted - no exceptions.

If you'd like to be a band parent/guardian chaperone at the festival, please complete both the Volunteer Application and Seattle School District Volunteer Screening forms.

Eckstein Band Together - Email List and Lab Donation Fee
The Eckstein Band Email List is used to communicate important band information to parents. The annual $25 lab donation helps support the band program.
Eckstein Band Deposit and Reimbursement Forms
To submit Eckstein Band checks or cash for deposit, complete both of these forms and return them to the Fiscal Officer, Patrice Holberg in the Main Office.
To submit reimbursement requests for band expenses, complete this form and return it to the Fiscal Officer, Patrice Holberg in the Main Office.
